1 # Intake presentation on candidal esophagitis 2 3 <time id="post-date">2022-03-25</time> 4 5 ## cc: trouble swallowing and weight loss 6 7 8 28M w few weeks of trouble swallowing (gets stuck "right here," points to mid-sternum), 9 solid/liquids same, 10 gradual over months-weeks, 11 some vomiting w/o specific timing. 12 Sometimes has pain when not eating. 13 20lb weight loss over months. 14 No skin lesions. 15 ?thrush. 16 17 ## PMHx/PSHx 18 19 dx BPD, no other dx or procedures 20 21 22 ## SHx 23 24 - MSM w occasional use of protection, no PrEP 25 - occ MJ use, no other substances 26 - unemployed, lives w mom 27 - no unusual hobbies or travel 28 29 ## PE 30 31 HR 100, SBP 80 -> 100 w 500mL LR, AF 32 cachectic (temporal, hypothenar wasting) 33 +skin tenting 34 diffuse abd tenderness 35 36 ## w/u 37 38 Hgb 10, MCV 88, WBC ~4, ANC 1500 39 BMP wnl 40 Alb 3.4 41 HIV+, VL 15k, rest of STI -ve 42 43 CXR wnl (AP and lateral) 44 45 ## dx 46 47 <p id="post-excerpt"> 48 candidal esophagitis, achalasia, H Pylori PUD 49 </p>
